For your convenience, our office sees patients by appointment. Please make your appointment well in advance so that we will be able to see you at the time that will fit your schedule. Please expect to fill out the Patient Information Form, a Medical History Form, and a Release Form that helps your physician to evaluate your problem. Every effort is made to see you at the time of your appointment. However, emergencies arise, which make this impossible. Your cooperation and understanding is appreciated.
